for the Prevention of COVID-19 Transmission for County Employees

 All persons entering the facility must submit to this screening. Please complete the form below.

  • Do you have symptoms of respiratory infection (fever over 100.4 F, newly developed cough, and shortness of breath)?
  • Have you been exposed to someone with CONFIRMED COVID-19? within the past 14 Days?

  • Please note, the list of countries that have a LEVEL 3 Travel Health Notice (Widespread, ongoing transmission) shown below may be updated in the future as the Centers for Disease Control frequently updates the transmission status designation of countries.
